Q. What is this website for? 
A.
This website allows you to pre-register for certain appointments at select Dignity Health facilities. 

Q. What are the benefits of Pre-Registration?
A.
Completing your Pre-Registration will save you time when you arrive for your appointment. 

Q. How do I register for an account? 
A.
When you schedule an appointment your invitation link and instructions will be sent to you by email or text message. 

Q. If my email invitation expires or is lost, what do I do? 
A.
No worries, if that happens we will have a pre-registration agent call you to register instead 

Q. My appointment has already passed. Should I still login? 
A.
You may login to your account at any time. However, the option to register for past appointments  will be disabled. 

Q. Can I make a one-time payment?

Q. How do I log into the portal? 
A.
Patients will log into the pre-registration portal using the following credentials: Username: Patient’s email on file at the facility where they will have service 

Password: Created by patient during pre-registration portal account setup 

Q. I can’t login who do I contact for help? 
A.
If you are in the Arizona market (St. Joseph's and St. Joseph's Westgate, and Chandler Regional and Mercy Gilbert medical centers) please contact the Pre-Registration department for assistance, 1-800-730- 5567. Note, all maternity patients: please contact the registration department at the facility where  you expect to deliver. 

All other patients should contact the registration department of the facility where your appointment is  scheduled to verify the following information: First Name, Last Name, Date of Birth, Medical  Record Number, and email address.
