Wound Care Physicians Referral

  1. Fax referral documents to 480.728.3702
  2. Please include:  demographic sheet, location of wound, photocopy of insurance card(s), any testing that has been completed and most recent physician note pertaining to the wound.
  3. We will call the patient to schedule appointment.
  4. If we have any questions, need more information or need pre-authorization, we will contact your office.
  5. On the referral, please state “PRN x 1 year” or “99 visits” since patients come back weekly until healed.


If Pre-Authorization is needed, please list:

  • Chandler Regional Medical Center
  • Doctor’s name (we will provide this for you)
  • CPT Codes (listed below)
