Connecting you to healthcare and social services.
Our elderly population in the East Valley have significant vulnerabilities that impact their health, their ability to access needed services and their quality of life. Participants enrolled in the Pathways to Wellness program will receive direct support from a CHW. The CHW serves as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. CHWs are trusted members of the healthcare community and can effectively lead participants through their health journey.
Program Features
- No cost, participant-led program
- Regular home visits from a licensed Community Health Worker (CHW)
What is Your Role
- Be engaged and willing to make health and/or social changes.
- Help define priorities and goals to address.
- Commit to a regular schedule for home visits with your CHW.
What to Expect
- You will partner with a CHW and meet at least once a month to identify the areas where you need support.
- Once you’ve identified your needs, your CHW will assist with care plan implementation, help develop care management strategies, and work with community resources to provide support for various health and social needs.
Eligibility
- 55+ or older;
- experiencing congestive heart failure, chronic disease such as Chronic lung disease, heart disease and comorbidities; such as diabetes,obesity, chronic respiratory, depression, High Utilization Groups (H.U.G.S), patients, Dignity Health - Home to Health patients;
- Resides in Maricopa County, East Valley hospital service area.
Insurance
AHCCCS, uninsured, underinsured are all accepted.
