Community Health Integration Network (CHIN)

Mission 

We believe in collaboration with organizations that share our values to provide health, social and support services to create a healthier community.  We strive to participate in community partnerships to improve the quality of life.  Through effective advocacy efforts, we address the social, political, and economic structures that impact the health of persons, especially those most vulnerable.  We seek to improve the health status of the community through focused, measurable efforts.  It is our role to “integrate the mission of Dignity Health - St. Joseph’s Hospital and Medical Center into the community and to engage the community in our mission integration.”

Purpose

The purpose of the Community Health Integration Network (CHIN) is to advise the Dignity Health - St. Joseph’s Hospital and Medical Center’s Community Board’s Community Benefit Committee as it relates to the program priorities based on the community needs and defined assets, to affect those programs, review, advise and recommend on technical and programmatic directions, and to evaluate the effectiveness of hospital’s programs.  This Advisory Committee will participate in the process of establishing program priorities based on the community needs and assets, review, advise, and recommend the Community Benefit Plan, Community Benefit Reports, and monitor the progress toward identified goals and strategic objectives that are established by the hospital.
