Application and Contact Information
Applicants must have a PharmD Degree from an ACPE-accredited school of pharmacy prior to starting the program. Applicants must also be a licensed pharmacist or eligible for licensure in the state of Arizona. Candidates with a cumulative pharmacy school GPA of 3 or higher (or top 50% of class for schools without GPA) are preferred. The average GPA among PGY1 candidates who matched to SJHMC over the past 3 years is 3.77 on a 4 point scale. Applicants for the PGY2 Critical Care Pharmacy Residency should have also completed a clinically oriented PGY1 residency in pharmacy.
A completed application packet must be submitted to PhORCAS by January 3rd, 2025. The application packet must include:
- PhORCAS application form
- Letter of intent
- Curriculum Vitae
- Official transcripts of all pharmacy education
- Three letters of recommendation (preferably at least two from clinical APPE preceptors)
