Donation and Sponsorship Request Application - Greater Phoenix Area

REQUEST FOR DONATION OR SPONSORSHIP

Dignity Health in Arizona is committed to supporting non-profit organizations for defined purposes that address community health needs that align with our mission, vision and values.  We prioritize donations and sponsorships that are restricted to a community benefit purpose as described on the application form.  

Organizations that address one or more of our identified community health needs will be given greater consideration.  Those identified community health needs are:

  • Access to Healthcare: Financial Security, Immunizations, Maternal/Child Health
  • Affordable Housing/Homelessness
  • Behavioral/Mental Health/Suicide
  • Cancer
  • Chronic Health Conditions: Cardiovascular, Diabetes, Obesity, Oral Health
  • Equity: Health, Racial, Social
  • Nutrition: Food Insecurity, Exercise
  • Substance Abuse/Addiction
  • Safety, Trauma and Violence: Domestic Violence, Human Trafficking, Injury Prevention

Some contributions may be considered “community building,” examples of which are outlined below. Charitable contributions that qualify towards community building as well as sponsorships/donations to arts/cultural organizations or events where a community benefit purpose is not evident will be given less weight when funding decisions are made. 

  • Physical improvements (to the community)
  • Economic development (for low-income, economically distressed areas)

  • Environmental improvements
  • Leadership development/training for community members
  • Coalition building
  • Community health improvement advocacy
  • Workforce development

We receive many requests for financial donations and sponsorships. While supportive in spirit of all approaches that encourage positive community improvements, we are limited in providing financial support to projects we view as most closely aligned with our core mission, values and addressing community benefit objectives.

COMMUNITY BENEFIT

Donations and sponsorship are given from the hospitals' operating budgets to non-profit organizations for defined purposes that address community health needs. The sponsorship oversight committee shall approve or decline each application based on compliance with guidelines and other strategic considerations.    

To Qualify

  • Requests should focus on activities or services that address one or more identified community health needs. We prioritize requests that seek to achieve a community benefit objective, including improving access to health services, advancing public health, advancing general health knowledge, and relieving the burden of government to improve health. The connection between health needs and the activity or service must be clearly stated in the application. Organizations that address one or more of our identified community health needs will be given greater consideration. Organizations that address one or more of our identified community health needs will be given greater consideration.   • Only 501(c) 3 organizations are eligible to apply. 

  • Geographic area: services supplied by requesting organizations should be available to residents of Arizona, preferably the Metro Phoenix area.  Organizations are advised to submit applications focused on projects, programs and activities supported by funds raised—and provided—in Arizona. An explanation of how funds will be used must be included.   

  • The following types of requests may be made under this application: donations that directly fund services, sponsorships of community events that raise funds for and that may directly deliver services, and in-kind contributions. 

  • Applications must demonstrate a commitment to provide services regardless of race, religion, sex, age, disability or national origin.

  • To avoid duplication, requests from national organizations should be coordinated between the regional and county affiliated offices within Arizona.

  • If the organization intends to submit multiple requests in the course of a one-year period, please submit all such requests separately but at the same time so we can best determine the request that fits our mission and health improvement goals.  

  • Event-related sponsorship applications must be submitted at least 75 days prior to the event in order to be eligible for consideration. 

We DO NOT Fund or Sponsor:

  • Partisan political groups/activities/campaigns, endowments, memorials, individuals, or tours
  • Requests from individuals or for-profit companies
  • Capital and Equipment
  • Programs that are inconsistent with our core values
  • Out of area organizations or most entertainment events
  • School or club sports or academic teams or activities including band, boosters, cheer, competitions, conferences, dance, field trips, tournaments, etc.  

SPONSORSHIP REQUEST APPLICATION

  • All requests must be made by completing this Google Request Form and email any additional documentation including  flyer, brochure, or sponsorship packages to [email protected].

  • Requests must be made at least 75 days prior to the date needed. The longer the lead time, the better. 

  • All requests are reviewed by the sponsorship oversight committee. The committee determines whether Dignity Health will sponsor your organization. Every applicant will receive email notification of the outcome of the request.  

  • Successful applications will receive payment within 4-6 weeks of receiving the documentation needed to process payment.    

  • After the Google Form has been submitted, please email the following documents to [email protected] using the Subject Format: Additional Documents for Donation Request | Name of Your Organization.
