For Patients – Appointment Information
At the Dignity Health – Cancer Institute at St. Joseph’s Hospital and Medical Center, we know that you have enough on your mind without having to navigate through a complex treatment plan. That’s why we aim to make your journey as smooth as possible, from the moment you contact us for your first appointment.
Making an Appointment
- Call us at (602) 406-8222 – our friendly staff members will help determine the best physician for you and will transfer you to the appropriate office.
- Please have the following information available when you call for your appointment:
- Your insurance card
- Your referring doctor’s contact information
- Your primary doctor’s contact information
- Information about your cancer diagnosis, including date of diagnosis, type of cancer, etc.
- One of our physicians may want to see pertinent medical records before your first appointment. These may include:
- Imaging/radiology studies
- Lab reports
- Pathology reports
If you do not have a copy of your medical records, we will help coordinate with your referring physician.
Preparing For Your Appointment
Once you have set your appointment, there are a few things you can do to ensure that your initial meeting is as productive as possible.
First, be sure that you know where your appointment will take place. The Dignity Health – Cancer Institute at St. Joseph’s is located at a beautiful, off-campus setting near downtown Phoenix:

We also recommend that you bring the following to your first appointment:
- Your insurance card
- Completed new patient paperwork
Instructions: Open form and use your tab key to type in required information. Print, sign and bring to your appointment.
- Conditions of Admission (English)
- Conditions of Admission (Spanish)
- Notice of Privacy Practices (NPP) (English)
- Notice of Privacy Practices (NPP) (Spanish)
- Your medical records, including lab reports, imaging/radiology studies and pathology reports
- A list of any medications you are taking, including dosage information
- A list of questions to ask your doctor
- A friend or family member to offer support and to help you remember important information
We can help coordinate with your referring physician for a copy of your medical records, if needed.
What to Expect
In addition to meeting with your physician during your first visit, you may also meet or receive information about other medical professionals dedicated to your cancer care. These may include:
- A registered nurse navigator to guide you through treatment and ensure access to emotional and social support resources
- A registered dietitian to offer nutrition counseling specific to cancer patients
- A genetic counselor may be asked by your doctor to meet with you to discuss genetic or family risk factors
- A clinical trial coordinator to help determine if you are eligible to participate in local or national trials for cancer treatment
- Supportive care and survivor services for symptom management and to help reestablish your quality of life after treatment
Please ask our office staff about making separate appointments with any of these individuals.
2nd Opinion Program
We also offer consultations for those who would like a second opinion on either their diagnosis or current treatment plan. Please call us to coordinate a second opinion consult.
