Nurse Navigators
Nurse navigators at the Dignity Health – Cancer Institute at St. Joseph’s Hospital and Medical Center are professional nurses with knowledge, skills and experience caring for individuals with cancer. They are trained to provide individualized and comprehensive care to help reduce barriers, coordinate diagnostic and treatment efforts and ensure prompt access to care.
Nurse navigators may also perform the following:
- Clinical assessments
- Education on disease process
- Explain plan of care and treatment therapies
- Help with symptom management
- Support patients through the complexity of care journey
Once you are assigned a nurse navigator they will help you collaborate and communicate effectively with your multidisciplinary team to effectively coordinate patient care, ensure continuity of care and create a comfortable and exceptional patient experience. They will also help with initiating referrals to our Supportive Care and Survivorship Program.
