Our Medical Records Department provides support to all patient care areas within the hospital. The staff is dedicated to making a positive contribution to the consistent high quality care to you before, during and after your stay at Dignity Health Sequoia Hospital.

Medical Records is open from 8:00 a.m. to 3:30 p.m., Monday through Friday. For more information, please call (650) 367-5586.

We Offer Three Options For Patients to Obtain Medical Records

  • Patient Portal

    Please be sure you have requested an invitation before accessing the Patient Portal. You can request an invitation by calling (650) 367-5586 or emailing seqroi@commonspirit.org.

  • Fax, Email, or Mail

    Step 1 
    Download, print and complete the authorization form. 

     

    Patient Request for Medical Records

     

    Release of Medical Records to Others

     

    The authorization form must be signed and dated. In order to verify your identification and validate your authorization, we require a legible copy of a valid photo I.D.

     

    Step 2
    Send the filled, signed, and dated form to the hospital via one of the following methods.

     

    1) Fax the form to (650) 482-6176

    2) Email the form to seqroi@commonspirit.org.

    3) Mail the form to the Medical Records Department.
