Helping You Manage Diabetes

Mercy Medical Center has a team of health professionals dedicated to providing you with support, education and the skills you need to take control of diabetes. We offer informative workshops, peer-led diabetes support groups, and clinical diabetes management services. We are here to help ensure you are receiving the information and support you need to manage diabetes successfully.

Clinical Diabetes Management Program

We work collaboratively with your primary care doctor in a “team approach” to support you and to provide you with the tools you need to manage diabetes on a daily basis. Our team consists of an Endocrinologist (doctor who specializes in Diabetes), Certified Diabetes Educators, and a Dietitian. You will need a referral from your primary care doctor to begin the program.

To help you get ready for your first visit to the Center for Diabetes, please:

  • Bring all your medications
  • Bring your glucose meter if you have one
  • Bring your food log if you have one
  • Think about how we can help you so you can best manage your diabetes
  • Write down and bring a list of your questions
