Accessing your medical records is easy using our online patient center. You can also download, fax or mail in your request for medical records.
Access your records online
Accessing We offer an online patient portal for you to access your medical records. You or a designated family member can view your medical records, download them, or send them to your healthcare provider. Please call us at (209) 467-6343 for proxy access or for help enrolling.
Visit St. Joseph's Medical Center records for access to patient records from before November 1, 2017. To access medical records from on or after November 1, 2017, please visit this portal page.
Requesting records via fax/mail
For access to billing-related health information aboutyou, and to enable us to process your request for a billing statement, please download the following forms.
- The Authorization for the Use or Disclosure of Protected Health Information form is for use when a patient is requesting that their records be sent to someone else (doctor, insurance, attorney, etc.).
- The Patient's Request for Access to Protected Health Information form is for patients or their representative who are requesting records on themselves.
Once complete, fax or mail form to the attention of the Health Information Management department. Please be sure to sign the form, unsigned requests cannot be processed. You can also email to: [email protected]

Please allow reasonable time to process your request. We will mail your medical records to the address specified on the authorization form. An invoice may accompany medical records requests.
