Preparing for Hospitalization
You have a right as a patient to make decisions about your medical treatment. You have the right to be fully informed about treatments and alternatives, including their benefits and risks to you. You have the right to consent or withhold consent to any health care option, including life-sustaining treatment.
In the event that you are unable to speak for yourself, your wishes can be preserved in advance by executing documents called Advance Directives for Health Care. Advance Directives can include a Health Care Power of Attorney or a Health Care Directive.
There are many options to choose from or you can find an Advance Directive form in English or in Spanish here.
