What is a Medical Record?
The medical record is a permanent document of the history and progress of every patient's medical care. The record is a compilation of observations and findings recorded by the patient's physician and members of the clinic health care staff. Records are used for continuity of patient care, to verify insurance claims for medical care rendered, and as a legal/business document outlining the course of a patient's medical care.

We offer an online patient portal for you to access your medical records. You or a designated family member (proxy) can view your records, download them, or send them to your health care provider.

Release of Information Request
Vital Records Control (VRC) provides release of information services on behalf of Pacific Central Coast Health Centers to better serve our patients. The estimated turnaround time is between three to seven business days following receipt by VRC of valid request and payment of any applicable fees.
What You Need to Know
For your convenience, patients can submit requests for medical records via mail, fax, or in person.

Completed forms can be submitted directly to VRC via fax at 805.614.2648 or completed forms can be mailed to:

Fees and Payments
Patients requesting medical records for personal use or sending to a third party will incur a
“processing fee” based on the following:
- The actual time needed to process the request.
- Media type.
- Delivery method.
- CA sales tax.
Payment is required before the delivery of records. VRC accepts Visa, MasterCard, and Discover. Notice of total charges will be provided in advance by VRC. There is no “processing fee” for medical records sent directly to another healthcare provider.
Delivery Methods
Completed requests can be delivered via electronic copy through VRC’s secure electronic portal, USPS mail, or other carriers.
