What is a medical record?
The medical record is a permanent document of the history and progress of every patient's medical care. The record is a compilation of observations and findings recorded by the patient's physician and members of the hospital health care staff. The entries and reports originate at various points in the hospital. Records are used for continuity of patient care, to verify insurance claims for medical care rendered, and as a legal/business document outlining the course of a patient's medical care.
Our Health Information Management (HIM)Department facilitates medical record release of information for patient care locations within the hospital, see listing below for applicable locations. Our staff is dedicated to delivering high-quality services before, during and after your stay.

We offer an online patient portal for you to access your medical records. You or a designated family member (proxy) can view your records, download them, or send them to your health care provider.

Requesting a Copy of Your Records for Yourself
To request a copy of your medical and/or billing records, download and complete the Patient Access Request Form. Once completed use the applicable Contact Us method to submit your request.
