Adult Day Health Care
Adult Day Health Care (ADHC) is a protective supervised daytime program providing skilled nursing, social services, and rehabilitative therapies, which includes recreational therapy, physical therapy, and occupational therapy for individuals with medical conditions and those at risk of requiring nursing home care. The goal of the program is to delay or prevent institutionalization by providing individual plan of care. Services include: medical care, medication management, assistance with personal care, door to door transportation, socialization, respite care, nutrition services, counseling, and education. ADHC centers are reimbursement by Medi-Cal and Veterans Administration.
Adult Day Program and Adult Day Healthcare (ADHC) are available to provide protective supervised care and socialization for adults who require assistance with activities of daily living.

Adult Day Program (ADP) is a non-medical adult day service, licensed by the CA Department of Social Services Community Care Licensing Division to provide care to adults with physical and/or cognitive impairments who require personal care services, protective supervision or assistance in activities of daily living on less than a 24-hour basis. Services are provided according to an individual plan of care in a structured, comprehensive program that offers therapeutic activities tailored to the individual's abilities; nutrition services; basic health monitoring; transportation coordination; and respite and support for families. ADP centers are not reimbursed by Medi-Cal or the Veteran Administration; this is done through private pay.

Caregiver Support Group
This monthly group is designed for caregivers who need resources, education, and emotional support. We invite anyone who is caring for someone with a disability or diagnosis that requires your support.
Each month, the topic and presenter will be different. All attendees will be given time to engage with peers who may be dealing with similar issues. Respite care will be provided as needed, free of charge.
We meet every third Wednesday of the month from 5:30 – 6:30 p.m. RSVP is required the Monday prior to the meeting by calling (530) 226-3000.
