Care Management services are designed to improve health, address daily needs, and foster wellbeing for people living with multiple health conditions and life difficulties.

Care Management services are designed for you. Whether it’s remaining living at home, receiving a nutritious meal, visiting with a friend or setting educational and employment goals, these services offer a foundation upon which recipients not only survive, but thrive.

  • Enhanced Care Management
    • Enhanced Care Management (ECM) provides a coordinated whole-person approach to care that addresses the clinical and non-clinical needs of Partnership Healthplan members. We are here to help you get resources and help you navigate through our challenging health system. Individualized goals will be set in collaboration with our Community Healthcare Workers. Participant goals are reassessed every six months with continual monthly contact. This is at no cost to you.

  • Multipurpose Senior Services Program (MSSP)
