Collaborative Partnerships
The mission-driven commitment of community health and outreach for the Sacramento services area includes hospital-sponsored programs as well as regional partnership efforts and initiatives. Our collaborative partnership programs exist to respond to the priority needs in our community such as preventing/managing chronic illnesses and improving access to mental health services, primary care and specialty care, and other resources that enhance community wellness and build capacity.
Programs that provide wraparound care and services, including transitional housing for those experiencing homelessness, substance abuse treatment, case management, safety resources for victims of domestic violence and human trafficking, resources that reduce the incidence of unintentional injury, and access to healthy food. We strategically focus on the prevention and management of chronic illnesses and health conditions prevalent in our region like; cancer, diabetes, stroke, heart disease. All of our partnership programs and services support the continuum of care needed for under-served populations in the communities we serve.
