Dignity Health's Home Health programs offers a full range of quality home health care options for patients recently discharged from the hospital, as well as homebound individuals throughout the Greater Sacramento region who need additional home care.
Our home health program also provides care for patients referred by doctors, insurers, hospitals and community agencies. Dignity Health proudly serves residents of Sacramento County, as well as parts of Placer, El Dorado and Yolo Counties. The Mercy Hospice program provides special care for terminally ill patients and their family members. Mercy Hospice provides physical, emotional and spiritual comfort and care in the supportive surroundings of the patient's place of residence. Our hospice program is certified and licensed to provide Medicare and Medi-Cal Hospice benefits, in addition to serving the hospice care needs of patients with private insurance.

Our animal-assisted therapy program, Source of Unconditional Love (SOUL, offers terminally ill patients a welcome respite.
