Yolo Adult Day Health Services
Woodland Memorial Hospital’s Yolo Adult Day Health Center offers a diverse range of services for individuals and families facing the challenges of serious health problems such as dementia, chronic medical diagnoses, mental illness or brain injury.
Our diverse programs to promote the well-being, dignity and self-esteem of an individual. Our goal is to maximize independence, improve management of chronic symptoms, prevent hospitalization and/or premature nursing home placement and provide support and relief to caregivers.
The professionally trained staff has a rich understanding of the issues related to community based chronic care management, caregiving, family dynamics, desire for independence, care coordination, home safety, community resources, memory loss and long term care planning.
Locations and Hours
The Yolo Adult Day Health Center is located at 20 N. Cottonwood St., Woodland. Our hours are Monday – Friday, 8:00 a.m. to 4:30 p.m. Drop in inquiry visits are welcomed.

We Assist Individuals and Families Who:
- Have trouble with everyday tasks
- Need assistance in managing medication
- Need ongoing physical, occupational or speech therapy
- Are isolated, depressed, confused or suffering memory loss due to disease or a related condition
Adult Day Health Care
Our medical model program provides a team approach to managing complex chronic health conditions that impact independent living. Our team includes physicians, nurses, social workers, rehabilitation specialists, recreation specialists, dietician and mental health professionals.
Adult Day Care
Our social model program offers program participants a stimulating and safe environment with a specialization in dementia care and family caregiver support. The center also provides specialized staff training to meet the needs of individuals with moderate to severe dementia who require additional assistance to benefit from our program.
Woodland: Caregiver Support Group
We offer a Caregiver Support Group for those caring for a loved-one with Alzheimer's Disease, Parkinson's Disease and other brain impairments. This group is a unique opportunity to discuss care-giving issues and strategies. Co-sponsored by the Alzheimer's Association.
Second Tuesday of each month, 5:30 to 7 p.m.

Program/Participant Cost
Medi-Cal, Veteran’s Benefits and some private insurance will cover the cost. Private pay rates vary. Call for cost information. Our daily fees include services, meals and transportation (transportation partially funded via Agency on Aging Area 4).

CalAim Enhanced Care Management Services
Dignity Health provides a community-based care coordination program called Enhanced Care Management (ECM). If you are a Yolo County resident and a Medi-Cal recipient with Partnership HealthPlan you could be eligible to receive ongoing support by our highly trained team of Community Navigators who can meet with you in your home or at a location of your choice.
Care coordination services include:
- Locate medical, dental, vision, and mental health providers and assist with making appointments.
- Coordinate transportation to get you where you need to go.
- Review support system at home: food security, cooking, cleaning, bathing and dressing.
- Help with budgeting, income and housing.
- Referrals to Partnership HealthPlan Community Supports for Medically Tailored Meals or Housing Navigation (if eligible).
