Northridge Hospital Foundation Receives $50000 Grant From Medtronic Foundation for Diabetes Wellness
The grant will allow Northridge Hospital’s Center for Healthier Communities to implement the Diabetes Wellness RX program to improve the health outcomes for Type 2 Latino Diabetic residents through culturally competent, patient-centered services. Funding supports the expansion of a Diabetes Self-Management Program, a peer support group, outreach and health screenings, physical activity, grocery store tours and cooking demonstrations.
“Northridge Hospital’s Center for Healthier Communities is in the final stages of completing our 2016 Community Health Needs Assessment and the community has prioritized Diabetes and Obesity as the top two concerns of community residents”, says Joni Novosel, Dignity Health – Northridge Hospital Director of Community Health. As part of a focus group for our Health Needs Assessment, 73.63% of the 91 community leaders and 375 community residents who participated, rated Diabetes as the leading number one issue facing our community. Support for the Diabetes Wellness RX program will help to decrease the escalating rates of Diabetes and improve health outcomes in our community.
