Fill out the form and fax it back to (562) 491-7965 with a copy of your valid driver's license or government-issued identification to the Medical Records Department, located on the 2nd floor of the main hospital.

Birth Certificates
Birth certificate information for a child less than one year old may be obtained by calling our Birth Clerk at (562) 491-9303 If the child is older than one, you will be referred to the Los Angeles County Hall of Records, Norwalk, CA which may be reached by calling (562) 462-2137. Or you may go online and order a certified copy of the birth certificate.
Privacy and Health Information
According to federal law Health Insurance Portability and Accountability Act (HIPAA), there are rules and limits on who can look at and receive your health information.This information includes your medical records and any other details on your illness and why you have been treated. We are committed to protecting your privacy. Without your written permission, St. Mary Medical Center can’t:
- Give your health information to your employer
- Use or share your health information for marketing or advertising purposes
- Share private notes about your mental health counseling sessions
