What is a Medical Record?

Our Health Information Management Department facilitates medical record release of information for patient care locations within the hospital. See below for applicable locations. Our staff is dedicated to delivering high-quality services before, during and after your stay.

View your records from your computer or mobile device:

  • We offer an online patient portal for you to access your medical (and billing, if necessary) records. You or a designated family member (proxy) can view your records, download them, or send them to your health care providers.
  • Visit the patient portal to access your medical records.

Request for a Medical Record

To request a copy of your medical and/or billing records, download and complete the Patient Access Request Form. Once completed, use the applicable Contact us method to submit your request.

We are prohibited from giving any protected health information (PHI) over the phone under any circumstances. 

Access Your Patient Records Online

Dignity Health now offers an Online Patient Center for our hospital patients. You or a designated family member can view your health records, download them or send them to your personal physician. Proxy access can be obtained at Health Information Management. For help enrolling, just ask us.

Visit DignityHealth.org/Patients and select St. John's Regional Medical Center from the drop-down menu to log on and access your hospital patient records.

Commercial record requests need to be sent via mail services or fax. (Please do not use the medical records email address for sending subpoena, attorney, or insurance requests.)
